Zepbound® (tirzepatide)

Intended Use:
Zepbound® (tirzepatide) is a once-weekly injectable prescription medication approved for chronic weight management in adults with:

  • A body mass index (BMI) ≥30 kg/m² (obesity), or
  • A BMI ≥27 kg/m² (overweight) with at least one weight-related comorbidity (e.g., hypertension, type 2 diabetes, obstructive sleep apnea, or dyslipidemia).
    It is to be used in combination with a reduced-calorie diet and increased physical activity.

Contraindications:
Do not use Zepbound® if:

  • You or a family member has ever had medullary thyroid carcinoma (MTC)
  • You have Multiple Endocrine Neoplasia syndrome type 2 (MEN 2)
  • You are allergic to tirzepatide or any of its ingredients

Warnings & Serious Risks:

  • Thyroid tumors, including cancer: In animal studies, tirzepatide caused thyroid C-cell tumors. Human relevance is uncertain.
  • Pancreatitis: Discontinue use and contact your healthcare provider if you experience persistent, severe abdominal pain, especially if radiating to the back or accompanied by vomiting.
  • Gallbladder disease: Watch for symptoms like upper abdominal pain, jaundice, or pale stools.
  • Kidney impairment: GI side effects can cause dehydration, which may worsen kidney function.
  • Hypoglycemia: Especially when used with insulin or insulin secretagogues.
  • Diabetic retinopathy: Monitor for any changes in vision.
  • Suicidal ideation or mood changes: Report behavioral or emotional changes promptly.
  • Severe gastrointestinal disease: Use is not recommended in patients with a history of severe GI disorders.

Drug Interactions:

    • Medications with narrow therapeutic indexes (e.g., warfarin)

Pregnancy & Breastfeeding:

  • Not recommended during pregnancy. Discontinue at least 2 months prior to planned conception.
  • Unknown if tirzepatide is excreted in breast milk. Avoid use while breastfeeding unless specifically advised by your provider.

Administration:
Inject subcutaneously once weekly in the abdomen, thigh, or upper arm. Administer on the same day each week, with or without food. Rotate injection sites with each dose.

Storage:
Refrigerate at 36°F to 46°F (2°C to 8°C). Do not freeze. If needed, Zepbound® pens can be stored at room temperature (up to 86°F / 30°C) for up to 21 days.

Emergency Situations:
Contact your healthcare provider or call 911 immediately if you experience signs of a serious allergic reaction, pancreatitis, low blood sugar, or persistent vomiting.
