Ozempic® (semaglutide)

Intended Use:
Ozempic® (semaglutide) is a once-weekly injectable prescription medication used to improve blood sugar (glucose) control in adults with type 2 diabetes, alongside diet and exercise. It may also be prescribed off-label for weight management in individuals with obesity or overweight and associated comorbid conditions.

Contraindications:
Do not use Ozempic® if:

  • You or a family member has a history of medullary thyroid carcinoma (MTC)
  • You have Multiple Endocrine Neoplasia syndrome type 2 (MEN 2)
  • You have a known allergy to semaglutide or any of the components in the formulation

Warnings & Serious Risks:

  • Thyroid tumors, including cancer: In rodent studies, semaglutide caused thyroid C-cell tumors. While relevance to humans is uncertain, avoid use in individuals with a personal or family history of MTC.
  • Pancreatitis: Discontinue and contact your healthcare provider if you experience persistent, severe abdominal pain.
  • Gallbladder disease: Watch for symptoms such as abdominal pain, yellowing of skin or eyes, or clay-colored stools.
  • Kidney problems: Dehydration from gastrointestinal side effects may worsen kidney function.
  • Hypoglycemia: More likely when used with insulin or sulfonylureas.
  • Diabetic retinopathy: May worsen in individuals with a history of this condition.
  • Suicidal thoughts or mood changes: Monitor for new or worsening mental health symptoms.

Common Side Effects:

  • Nausea
  • Vomiting
  • Diarrhea
  • Constipation
  • Abdominal pain
  • Headache
  • Fatigue
  • Indigestion
  • Injection site reactions

Symptoms typically lessen over time as your body adjusts to the medication.

Drug Interactions:

  • Insulin and sulfonylureas: May increase risk of hypoglycemia.
  • Oral medications: Delayed gastric emptying may alter absorption; monitor any changes in therapeutic effect, especially with narrow therapeutic index drugs.
  • Oral contraceptives: Discuss with your provider whether backup contraception is needed when starting or adjusting dosage.

Pregnancy & Breastfeeding:

  • Ozempic® should not be used during pregnancy. Discontinue at least 2 months before planning conception.
  • It is unknown whether semaglutide passes into breast milk. Discuss risks and benefits with your provider before using while nursing.

Administration:
Inject subcutaneously once weekly in the abdomen, thigh, or upper arm. Administer on the same day each week, with or without food. Rotate injection sites with each dose.

Storage:
Refrigerate (36°F to 46°F / 2°C to 8°C). Do not freeze. If necessary, pens can be kept at room temperature (up to 86°F / 30°C) for up to 56 days.

Emergency Situations:
Seek emergency care if you experience signs of severe allergic reaction, persistent vomiting, severe abdominal pain, or low blood sugar. Call 911 or visit the nearest emergency facility.
